Standard IVF Tests Overlook Certain Genetic Abnormalities in Embryos

During in vitro fertilization (IVF), embryos are subjected to genetic screening prior to being placed in the uterus. Recent studies, however, have shown that the common tests may fail to identify genetic abnormalities arising shortly before implantation. The implications for choosing embryos that are likely to lead to a healthy pregnancy remain uncertain.

This process, known as preimplantation genetic testing for aneuploidy (PGT-A), is conducted about 5 to 6 days after fertilization. It involves extracting cells from the embryo’s outer layer to assess for chromosomal irregularities, which can elevate the risk of miscarriage. However, this testing only captures a moment in time, as cellular division continues and may introduce genetic changes prior to implantation.

To address this gap, Ahmed Abdelbaki and his colleagues at the University of Cambridge monitored the progress of human embryos 46 hours post-thawing, replicating the timeline from evaluation to implantation. Typically, the embryo takes 1 to 5 days to implant after being transferred to the uterus. Given that embryos are highly sensitive to the light from traditional microscopes, prior studies only managed to observe them for about 24 hours. The research team employed light-sheet microscopy, a technique that illuminates only a thin slice of the embryo at once, minimizing light exposure and enabling longer observation durations.

In their experiment, the researchers injected 13 human embryos with a fluorescent dye that attaches to DNA, facilitating real-time tracking of genetic abnormality formation. They recorded the division of 223 cells and discovered that 8% exhibited chromosomal misalignment. This misalignment occurs when chromosomes improperly arrange themselves before cell division, significantly raising the likelihood of creating cells with abnormal chromosome counts, potentially hindering implantation, increasing miscarriage risk, and leading to conditions such as Down syndrome.

This indicates that genetic changes might arise later. “These variances appear in the embryo subsequent to PGT-A screening,” stated Lily Zimmerman from Northwell Health in New York.

These chromosomal errors were restricted to the outer cell layer responsible for forming the placenta, rather than the central cells that mature into the fetus. Previous findings suggest that successful pregnancies can occur even with certain genetic abnormalities in the outer cells. Thus, Abdelbaki posits that these genetic errors may not detrimentally impact the embryo’s survival chances.

“In my view, this study highlights the necessity for further research in embryo screening. It’s not simply a matter of categorizing embryos as genetically normal or abnormal,” commented Professor Zimmerman. She also noted that it remains unclear how genetic alterations occurring between screening and implantation might influence embryo viability, and given that the study examined only a small sample of embryos, the broader applicability of these findings is uncertain.

Urine Tests Identify High-Risk HPV as Effectively as DIY Vaginal Swabs

Urine tests can detect strains of human papillomavirus (HPV), linked to cervical cancer, with accuracy comparable to that of self-administered vaginal swabs.

Historically, cervical cancer screening involved healthcare professionals collecting cell samples from the cervix to test for abnormal cells or HPV strains. HPV 16 and 18 are the most common culprits.

In nations like the US and Canada, individuals can now opt for self-collected vaginal swabs that avoid direct contact with the cervix. This approach is similarly being adopted in the UK as part of a new strategy. Such swabs have been linked to increased screening uptake, although they may still be invasive and uncomfortable for some individuals.

Previous research indicates that HPV DNA can be identified in urine. To explore this further, Julia Lynch from the International Vaccine Institute in Seoul, along with her colleagues, surveyed 753 sexually active women aged 18-25, asking them to provide urine samples at any time and to perform vaginal swabs in clinical settings across Bangladesh, Pakistan, and Nepal.

The results revealed that the capacity of the tests to identify seven high-risk HPV strains was strikingly similar, with 5.3% of self-collected swabs testing positive for one or more of these strains compared to 5% of urine samples. For HPV 16 and 18 specifically, the figures were nearly identical—2.3% for swabs and 2.4% for urine.

Urine samples were also preferred by many participants. “We operate in regions with diverse social contexts, and vaginal swabs were less accepted among certain age groups in some countries,” Lynch notes.

The World Health Organization set elimination goals for cervical cancer in 2018, primarily hinging on the success of HPV vaccinations. However, maintaining screening uptake remains critical, as the effects of vaccination programs may take years to be evident on a population level, according to Lynch.

Several types of HPV vaccines protect against all strains 16 and 18. Nonetheless, the data concerning HPV risks generally originates from North America and Europe, as Lynch states. Thus, this research could provide insights into the circulating strains in various regions globally to inform vaccine strategies.

The study focused solely on young women, which means the findings may not apply to broader demographics. Beverly Green from Kaiser Permanente Washington Health Research Institute mentioned that the current study is a part of a wider project aimed at determining HPV prevalence in eight low- and middle-income countries throughout South Asia and Africa, with additional studies within the same project assessing a sample of older women.

An intriguing finding arose from a challenging task involving a sealed box where puppies attempted to retrieve treats. Interestingly, those exposed frequently to nearby humans were often more likely to fear unfamiliar humans as adults, even if they were strangers.

“I’m just trying to focus on what I can control,” U.S. triathlete Kirsten Kasper told NBC News on Tuesday. “We swim in a lot of cities and water quality is often an issue, but I just have to trust that the race organizers are doing the testing and doing what it takes to make sure we’re safe.”

Water experts said the difficulty of keeping the Seine clean enough could draw attention to a broader problem of environmental pollution shared around the world.

“In large cities, it’s very difficult to control the amount of human waste that you see,” said Katie Graham, an assistant professor in the Georgia Institute of Technology’s College of Engineering. “The public assumes that a lot of these problems have been completely solved, but that’s by no means the case.”

UK AI Safety Association: Setting Standards, Not Tests, is Essential for Artificial Intelligence Safety

The UK should prioritize setting global standards for artificial intelligence testing, instead of attempting to conduct all reviews itself, as suggested by the company responsible for the government’s AI Safety Institute.

Mark Warner, CEO of Faculty AI, emphasized the institute’s commitment to AI safety and its development of technologies for chatbots like ChatGPT. He cautioned that excessive scrutiny of AI models could be limiting.

Last year, Rishi Sunak announced the establishment of the AI Safety Institute (AISI) ahead of a global AI safety summit. This initiative involved collaboration with large tech companies from the EU, UK, US, France, and Japan to prioritize testing of advanced AI models before and after deployment.

The UK’s leading role in AI safety was underscored by the establishment of the Institute, according to Warner, whose London-based company also works with a British lab to test AI model compliance with safety guidelines.

Warner stressed the importance of the institute becoming a global leader in setting testing standards: “I think it’s important to set standards for the wider world rather than trying to do everything ourselves,” he said.

He also expressed optimism about the institute’s potential as an international standard setter, promoting scalability in maintaining AI security and describing it as a long-term vision.

Warner cautioned against the government taking on all testing responsibilities, advocating for the development of standards that other governments and companies can adopt instead.

He acknowledged the challenge of testing every released model and suggested focusing on the most advanced systems.

The Financial Times reported that major AI companies are urging the UK government to expedite safety testing of AI systems. Notably, the US also announced the establishment of an AI Safety Institute participating in the testing program outlined at the Bletchley Park summit.

The UK’s Department for Science, Innovation and Technology emphasized the role of governments in testing AI models, with the UK taking a leading global role through the AI Safety Institute.
